Fix exec hang after Bash timeout

Summary

• prevent headless exec from hanging when terminal turn completion events are missed
• make terminal lifecycle event emission best-effort so undrained channels do not block tools
• close timed-out Bash terminal sessions and terminate Unix PTY process groups
